The 2012-2013 SES provider application is closed.  The MDE SES online provider application will be open for recertification of your organization’s last approved application from 8:00 a.m., June 1, 2012 through June 29, 2012, and will close on that date at 5:00 p.m.  All currently approved SES providers must recertify to remain on the MDE approved provider list.   All MDE approved SES Providers must re-certify their last approved online applications in June 2012, and make only changes already approved in writing by MDE for the 2012-13 school year in the application in Section B.  Even if the already approved SES provider has submitted a new application that remains under review for “substantial changes” requested, pending approval or denial, the provider must recertify the last approved application.  This is because the approval status of new applications with “substantial changes” may not be known until July 1 or later, after closure of the recertification window.   Do NOT make changes that have not already been approved by MDE.  Changes to Section A, Basic Program Information, #6 and #7, Contact Information, should be made as necessary by providers recertifying.
